To start with, surf Google forms website & press on create a form.
Here you will find two options of How to add google forms to WordPress- build a new form from scratch or choose from any pre-defined templates.
For example- lets choose Course Evaluation Form, instead you can select anything you want.
There is a floating menu at the side that helps you to make changes & customization the form to great extent. You can add more titles, questions & description, insert pictures or videos and create many other sections.
Questions may display in the different formats like MCQs, drop down or linear scale and you can drag & drop questions to manage them in the desired order. Also you can select a visual design. Besides if you add your logo or photo, Google automatically comes with a options of colors or themes that suits your template.
You can also apply skip logic to your questions, customize confirmation page, use add-ons for more powerful form.
Once you configured everything, find the eye icon at the top to preview your form. If you like this then go ahead otherwise make the desired changes. Now click the send button.
